Northern Health Health Independence Program (HIP)is seeking an exceptional leader to coordinate the Hospital Admission Risk Program (HARP).
The successful applicant will be required to have strong leadership and change management skills in order to
facilitate the transition of HARP to the integrated HIP program. The Coordinator must ensure that HARP delivers safe, cost effective services including care coordination, and will be responsible for operational management and performance of the program.
