You will have a sound clinical background and an interest in evidenced based care and care coordination. Your role will include providing focused, comprehensive assessments, coordination of individualised care plans and specific health discipline interventions.
The position will be based at one of the Northern Health campuses in Epping, Bundoora and Broadmeadows.
