About the Organisation
The Eastern Sydney Medicare Local (ESML) is a well-established organisation which oversees the provision of primary health care services to communities in the CBD, eastern and south eastern suburbs of Sydney, between Sydney Harbour and Botany Bay.
The Eastern Sydney Medicare Local will build on its relationships and linkages with the Local Health Districts, Local Governments, allied health and primary care providers, a network of community based service providers, the Aboriginal and Torres Strait Islander Forum, and Association of Community Nurses and Nurse Practitioners. The Eastern Sydney Medicare Local will work with key health and community stakeholders across the catchment in the establishment of the Medicare Local.
About the Opportunity
Eastern Sydney Medicare Local has an exciting opportunity for a Team Leader - Care Coordinator to join its dedicated team in Rosebery.
Reporting to the Director of Primary Health Care, you will be responsible for providing supervision and mentorship to the ESML Care Coordinators (Connecting Care and Care Coordination and Supplementary Services Programs) and for ensuring the ESML care coordination programs meet their objectives and team members meet targets.
